PREPARING FOR YOUR COLONOSCOPY OR SURGERY

Based on your individual situation, your physician may suggest that you follow one of the diets below.

In order for the physician to get a clear view of the lining of the colon, various bowel preps that are used to thoroughly clean the colon of all residue. Your physician will choose the appropriate bowel prep for your procedure and give you specific instructions to follow. You may print out specific instructions on these prep sheets, and insert times and dates in blanks as instructed by your physician:

Instructions for Downloading Forms

  1. Right-click the file name/hyperlink. 

  2. Select Save Target As.

  3. Choose the location on your computer in which to save.

  4. Open the file after it's downloaded.
